Kumail gets a call, while he's in bed with another comedy groupie, that Emily is sick. He rushes to the hospital, and pretends to be her husband, under duress from a doctor, so that he can sign an urgent release form to put her into a medical coma.
He soon meets her parents, who don't like him very much and don't want him around.
But Kumail, bless him, is persistent. Slowly Emily's parents bond with him, her dad even opening up to Kumail one night that he cheated on Emily's mother and deeply regrets it.
Meanwhile, Kumail opens up to his parents that he doesn't want an arranged marriage, causing his family to (pretty harshly) disown him.
Emily fights the infection, and the hospital finally figure out what is wrong with her. She wakes up, but doesn't want to see Kumail any more.
Heartbroken, he makes his way to New York to try and earn a better living as a comedian.
Later, at one of his shows, a heckler shouts out from the audience - it's a much happier Emily, in a scene that echoes the one where they met (that old movie 'Book end' chestnut), and actually to be fair it's the highlight of the whole movie.
It ends with photos of the real Emily, and Kumail's family. Who now seem to have forgiven him, no doubt helped by the fact that he is now a mega rich and famous TV and Movie Star.
